    Background: Xerostomia forms one of the most common manifestations of systemic diseases or occurs as an adverse effect of medication. Treatment of xerostomia involves both in minimizing the symptoms and reducing the oral complications by administration of sialagogue such as pilocarpine and bethanechol which are cholinergic drugs in this study. Aim: The aim of the study was to compare the efficiency of pilocarpine and bethanechol as effective sialagogue agents in completely edentulous xerostomic patients. Materials and Methods: A closed label randomized control trial study was conducted among 30 xerostomic complete denture patients who reported to the clinic. Whole resting saliva (WRS) and whole stimulated saliva (WSS) were collected for 2 min in a test tube following which the sialagogue drug is administered for 2–3 weeks course and the results were statistically analyzed. Results: On statistical analysis, there was significant increase in mean salivary production of both the drugs that are pilocarpine and bethanechol, respectively, during the 1st week (P < 0.001). On crossover, pilocarpine was found to be more efficient than bethanechol with respect to both WRS and WSS, respectively (P < 0.001). Conclusion: This study shows pilocarpine on comparison with Bethanechol is more effective in the management of xerostomia in completely edentulous patients.
