Aim: The aim of this systematic review is to analyze the existing literature on the role of spirulina in the treatment of  oral submucous fibrosis (OSMF). Materials and Methods: A comprehensive search of the electronic literature databases (MEDLINE/PubMed, Scopus, ISI Web of Knowledge, and Google Scholar) was carried out to identify all relevant studies published up to December 31, 2018. Various combinations of the following keywords were applied: OSF, oral submucous fibrosis, idiopathic scleroderma of the mouth, juxta‑epithelial fibrosis, Asian sideropenic dysphagia, spirulina, and herbal therapy. Titles and abstracts of all the retrieved articles were screened, and irrelevant studies were excluded. Full texts of the potentially relevant studies were obtained and thoroughly assessed for inclusion. Manual search was also done to rule out additional studies. Results: The initial search yielded a total of 12 studies. Following an initial screening of the titles and abstracts, four studies were found irrelevant and were subsequently removed. The full texts of the remaining seven studies were sought and thoroughly read and assessed. Two of these studies were excluded due to the exclusion criteria. Ultimately, the five studies met the inclusion criteria and were included in this study. The five studies assessed parameters such as mouth opening, burning sensation, cheek flexibility, and tongue protrusion. Conclusion: From the five clinical trials, we can conclude that spirulina is an effective treatment in managing oral submucous fibrosis. Although only five clinical trials have been done, further clinical trials are necessary to prove the efficacy of spirulina in managing oral submucous fibrosis.
